Why one specimen has two numbers
- A laboratory measures one thing: the concentration of glucose in plasma. Everything below is a change of unit, not a change of measurement.
- mg/dL is a mass concentration — milligrams of glucose per decilitre. India, the United States and much of Asia report it.
- mmol/L is a molar concentration — millimoles per litre. The UK, Australia, Canada and most of Europe report it.
- The bridge between them is the molar mass of glucose, 180.16 g/mol, which in these units is 18.016. The clinical shorthand of “divide by 18” is accurate to within a rounding error across the entire reporting range.
- No cut-point moves when the unit does. 126 mg/dL and 7.0 mmol/L are the same threshold written twice.
