The conversion
mmol/L = mg/dL ÷ 18.016 · mg/dL = mmol/L × 18.016
The factor is the molar mass of glucose, 180.16 g/mol, expressed in the units the two scales use. Almost everyone rounds it to 18, which is accurate to within a fraction of a unit anywhere on this page.
